Report unsafe work in WA: WorkSafe contact options

Short answer

In Western Australia, WorkSafe recommends trying to resolve safety complaints on site before reporting them. For hazards that aren’t life-threatening, you can phone or email WorkSafe. Anyone can report, though reports from people who saw it themselves get higher priority. You can ask WorkSafe to keep your contact details confidential.

Start on site

WorkSafe WA recommends reporting and trying to resolve workplace safety complaints internally before it gets involved. Where talking with the supervisor, the PCBU or the health and safety representative hasn’t fixed it, the next step is WorkSafe. The more of that conversation you can show, the easier it is for an inspector to see what has already been tried.

Going to WorkSafe WA

For a hazard that isn’t life-threatening, WorkSafe has a phone number and an email address, both in the table below. WorkSafe wants to know who is involved, what is unsafe, where and when, how you know, and what consultation has happened. Anyone can report, but a report from someone who saw it themselves gets higher priority than something passed on. An inspector may not investigate every complaint.

Your name and what happens to it

WorkSafe gives you a reference number when you leave contact details, and you can ask it to keep those details confidential. It also says that in some cases it can’t investigate unless it can contact you. For complaints about inappropriate behaviour, WorkSafe says it can’t guarantee your anonymity and an investigation needs your consent.

When someone has been hurt

A death, or a serious incident that is life-threatening, is a different call. WorkSafe asks for an immediate phone call on its emergency number. Our Western Australia notifiable incidents page has the detail, and the last row below carries WorkSafe’s wording.

Where Haznote fits

Haznote helps with the “how do you know” question. A report from the worker who saw it, with their voice, a photo and a GPS point, comes straight from the person who saw it. The manager confirms it and it stays on the record. Haznote doesn’t contact WorkSafe, and anonymous reports are a site setting that’s off by default and best-effort.
