Report unsafe work in South Australia: SafeWork SA
Short answer
In South Australia, SafeWork SA expects reasonable steps to resolve a concern in the workplace before you raise it with the regulator. If that fails or isn’t suitable, you can call its advice line or complete the concern form. Inspectors don’t reveal who reported without consent, and SafeWork SA allows an anonymous report.

Start on site
SafeWork SA is specific about what comes before it. Raise the matter verbally with a supervisor or manager. Use the workplace’s own hazard reporting, speak to a health and safety representative, or go through a union representative. Any of those counts as a reasonable step. Write down which one you used and when, because the regulator may ask.
Going to SafeWork SA
If those steps fail or aren’t suitable, ring SafeWork SA’s advice line on weekdays, or complete the report form on its site. To act, the regulator needs the address and the spot on site where the problem is. It also needs the kind of problem, such as work at height, and who controls the workplace. A clear photo and a pin on the map fill most of that in.
Your name and what happens to it
Inspectors don’t reveal the source of a report to any of the parties unless you consent. SafeWork SA also allows an anonymous report. The cost is that inspectors can’t ask you for more detail or give you feedback. For a small job, work out whether the circumstances point at you anyway.
When someone has been hurt
For a death or a life-threatening injury, SafeWork SA has a separate statewide number that takes calls around the clock. Our South Australia notifiable incidents page covers it, and the last row below carries the regulator’s own words.
Where Haznote fits
Haznote suits the “reasonable steps” trail. The worker’s report, the supervisor’s response and the date all sit on one record, which is easy to show later. It won’t lodge the form for you. Anonymous reports are a site setting, off by default, and best-effort when switched on.
What the regulator says
Each row shows the source’s own words, who published them and when they were last checked by script.
