Short answer

In South Australia, a death or life-threatening injury goes to SafeWork SA on 1800 777 209, any hour. Other injuries go to 1300 365 255 or the notifiable incident form. The 2025 model amendments have no legal effect in South Australia until they’re formally adopted.

What counts, and who to call

SafeWork SA separates notifications by urgency. A death or a life-threatening injury uses a statewide 24-hour number. A serious injury or illness that is not life-threatening, or a dangerous incident, uses a business-hours number or the online notifiable incident form. The regulator lists the same three triggers as other harmonised states: fatalities, serious injuries and illnesses, and dangerous incidents.

How SA differs from the model law

South Australia uses the harmonised triggers. What differs right now is timing. SafeWork SA says the model notification amendments published in December 2025 take no legal effect in South Australia until they are formally adopted into its legislation. So the current South Australian definitions apply. Notification runs on two numbers, a statewide 24-hour line for a death or life-threatening injury and another for other injuries, or the notifiable incident form.

A worked example

A press brake takes off two fingertips at an Adelaide workshop and the worker goes to hospital. The owner calls SafeWork SA on the statewide line because the injury is serious, and leaves the press brake as it is. The notifiable incident form is filled in the same day. The inspector asks for the history. That brings up a near miss on the same machine two weeks earlier, with the notes made at the time.

How Haznote handles it

For a South Australian site, Haznote names SafeWork SA on the flag. The manager confirms it. The earlier near miss on that machine is easy to find, because reports stay on file and are never deleted.
