Hazard reporting that works for site supervisors

Short answer

A supervisor needs to log a hazard in the time it takes to walk away from it. With Haznote you record a short voice note, add photos, and send. Location and time are attached, and with no signal the report waits on the phone and goes out when the connection returns.

Automatically checked against SafeWork NSW and WorkSafe WA on 31 Oct 2026.

Assign the fix, close it with evidence, and the worker sees it was fixed.

The job between the job

Supervisors see a hazard early and have the least time to write it up. The form on the clipboard waits until smoko, then until the end of the day. By then the detail’s gone. Which stair, which edge, who was standing there. A voice note keeps it because you make it at the spot.

What happens when something serious occurs

When an incident is serious, the opening minutes are about people, then about the scene. Each state’s regulator says to keep the site as it is until an inspector arrives or gives direction, with room to help the injured and make the area safe. The rows below show two states in the regulator’s own words.

Haznote doesn’t replace that judgement. It takes the paperwork off the next few minutes. Record what you see and hear while the scene is held, add photos from where you stand, and the report is time-stamped with your location. Once it’s sent, someone at the desk can see it. If you remember something later, you can add it to the same report.

Made for the field

  • Large, high-contrast controls
  • Photos taken inside the report, not in a separate app
  • Reports queue on the phone and send when there’s signal
  • The worker’s original words travel with the AI’s draft

Questions

Plain answers.

Do I need a smartphone app from a store?

Workers use an installable web app that opens from a link or the site QR code. A manager can invite a worker, or the worker joins the site through the site’s QR code.

What if I record something wrong?

Record again, or add a supplement to the same report. Reports are marked, never deleted.

Will my voice note be shared widely?

Reports go to the managers for that site. A confidential report goes to the company’s confidential contact instead of site managers.
