A WHS incident PDF with the worker’s own words
Short answer
A manager can turn a report into a General Incident Report PDF. It shows when the incident occurred and when it was received. The AI’s description sits beside the worker’s own words, followed by the WHS category, actions, a corrective actions table and the status history. Every page has an AI disclosure.
What the document is for
The insurer, the principal contractor or an inspector asks what happened, and a stack of texts won’t cut it. The General Incident Report puts one incident in one document. The layout is the same every time, so the reader knows where to look.
What is in it
- Company legal name, ABN and address, the report number and when the PDF was generated
- When the incident occurred and when it was received, as separate rows, plus status, site, zone, priority and GPS
- The AI’s formal description and the worker’s original words in separate sections
- WHS category and code, media, immediate action, root cause, risk score, remedial actions
- A corrective and preventive actions table and the status history
Nothing in the document claims more than it is. The description is headed by where its text came from. On an AI-assisted report, the footer on every page says so, and says whether a person has reviewed the draft. The status history lists each change with its date, the person and their role. That includes a manager overriding the priority.
Limits, stated plainly
The PDF records what was entered. It doesn’t notify a regulator, and it isn’t a legal opinion. If a company has no ABN on file, the PDF says so instead of leaving the line blank. The PDF carries the photos attached to the report and the status history, with who changed what and when. Want to see the layout before you decide anything? The sample WHS PDF page shows it, built from example data.

Plain answers.
Does the PDF notify the regulator?
No. Generating it doesn’t notify anyone.
What if our ABN isn’t recorded?
The PDF says the ABN is not yet recorded, so the gap is visible.
Can I see one?
Yes. The sample WHS PDF page shows the layout from example data.
Where this comes from.
