Report unsafe work in Queensland: WHSQ inspector
Short answer
In Queensland, the route to Workplace Health and Safety Queensland for an unresolved issue is a request for an inspector, made by phone. Reasonable efforts to settle it on site come before that. Just the parties to the issue can ask, so work out who that is on your job before you ring.

Start on site
Sort it out on site where you can. Queensland’s inspector route expects that you’ve already tried to settle the matter with the business, and the regulator will ask what you tried. A written report helps here. Date, place, photo and the name of the person you told. That trail is also what you read out when you ring.
Going to Workplace Health and Safety Queensland
To ask for an inspector, phone Workplace Health and Safety Queensland on the number in the table below. You’ll be asked for details of the issue and the business or undertaking, with its ABN if you have it. You’ll also give contact details for the parties, and your role. Having those to hand makes the call short. The regulator’s page limits who can ask to the parties to the issue.
Your name and what happens to it
Be clear about what this route is. It is built around parties to an issue who give their contact details. So it isn’t a way to raise a concern without your name on it. If naming yourself is the problem, talk to your health and safety representative or union before you ring. Ask the regulator what other routes it has.
When someone has been hurt
Hazards and incidents travel different roads. A death, a serious injury or a dangerous incident is notified to Workplace Health and Safety Queensland straight away, by phone, online or by email. Our Queensland notifiable incidents page has the steps, and the last row below shows the regulator’s own text.
Where Haznote fits
For a Queensland job, Haznote gives you the trail the inspector route asks about. The worker’s report, the manager’s reply and the time between them all sit in one list. It doesn’t ring the regulator or request an inspector. Anonymous reporting is a site setting, off by default and best-effort.

Plain answers.
Who can ask for an inspector in Queensland?
Parties to the issue, in the regulator’s words. Who counts as a party depends on the situation, and WHSQ’s page explains it.
What should I have ready?
Details of the issue, the business including its ABN if known, contact details for the parties, and your role.
Is a Haznote record enough on its own?
No. It’s a record of what was reported and when. The regulator decides what to do with a request.
