Hazard report form for farms and agriculture

Short answer

A farm hazard form deals with distance and isolation: which paddock, how far from help, whether anyone knew where you were. It also covers tractors, PTO guards, animals and chemicals. This page lists the fields, with a free PDF and Word template and a voice version that works with no signal.

What goes wrong in agriculture

On a farm the nearest person is often a long way off, and the hazard might be a bull, a bin or a bonnet. The form leans on location and isolation, since a report from the back paddock is no use if nobody can find the paddock.

  • Tractor and ute rollovers on slopes and washed-out tracks
  • PTO shafts and belts without guards
  • Livestock handling, especially cows with calves and bulls in yards
  • Spray drift, and chemicals stored without labels
  • Grain bins and silos where a person can be buried
  • Working alone without a check-in

Fields that earn their place on a agriculture form

Farms name their ground, so the paddock name and the distance from the house do the work an address does in town.

  • Paddock, shed or yard name, and distance from the house
  • Machine or implement, and whether the PTO guard was on
  • Animal behaviour and the state of the yards or crush
  • Chemical in use and the re-entry period on the label
  • Whether the worker was alone, and the last check-in
  • Phone coverage at the spot

On paper and by voice

You’re moving cattle in the back paddock when the quad slides on a washed-out track. You’re fine, but the track is a trap. The nearest bars are on the hill. You record “back paddock, creek crossing, track washed out, nearly rolled the quad” and take a photo. The report sits on the phone until you reach the hill.

Where Haznote fits

Haznote saves the report on the phone and sends it when coverage returns, with the location from the phone’s GPS. The manager confirms the category and priority. It doesn’t alert anyone if you fail to check in, so that stays a separate arrangement on the farm.

Questions

Plain answers.

We’re a family farm. Is this for us?

It works for any size. If you employ workers or contractors, a dated record of hazards and fixes is useful.

What about a quad bike or tractor incident?

Use the vehicle incident template. This form is for the hazard before anyone is hurt.

Can workers report without a signal?

Yes. The report stays on the phone until there’s coverage.
