Hazard report form for plumbers and gasfitters

Short answer

A plumbing hazard form covers things other trades rarely meet. Gas that smells wrong, hot work near lining, pits and tanks, and old lagging that might be asbestos. It asks which system, whether it was isolated and who was told. Download the PDF or Word template, or fill it by voice.

What goes wrong in plumbing

Plumbers work in roof spaces, trenches, pits and under floors, and the hazard is often behind a wall. The form needs to say which system was involved, because a gas problem and a stormwater problem are different jobs for different people.

  • A gas smell in an under-floor or enclosed space
  • Soldering or brazing near timber, lining or insulation
  • Pits, tanks and sumps where air can run short
  • Old pipe lagging that looks like it might contain asbestos
  • Hot water units and heavy cast iron lifted awkwardly
  • Sewer work with splash, sharps or confined access

Fields that earn their place on a plumbing form

A plumber’s report goes to a supervisor who may not know the building. Name the system and where the isolation valve is, and they can act without a site visit.

  • System involved: gas, hot water, cold water, sewer or stormwater
  • Fixture, room or pit where it happened
  • Isolation valve location and whether it was closed
  • Suspect lagging or fibre sheet seen, with a photo and work stopped or not
  • Hot work in progress and whether a fire watch was posted
  • Pit, tank or confined entry, and the permit number if one was used

On paper and by voice

You’re cutting into a 1960s riser when you notice grey wrap on the pipe that nobody has mentioned. Your hands are wet and you don’t want to touch the phone screen. By voice you say what and where, snap the lagging from a metre away and stop work. The record shows the time you stopped.

Where Haznote fits

Haznote attaches the photo and a location to the voice note, and the manager gets the report with your words next to the AI’s draft. It doesn’t identify materials from a photo. A person and, where needed, a licensed assessor decide what the material is.

Questions

Plain answers.

Is there a separate gas form?

Use this one and set the system field to gas. A suspected leak gets a phone call as well as a form.

Can I report old lagging I just suspect?

Yes. Say what you saw and where. A suspicion with a photo is more useful than silence.

Who is the form for?

The site supervisor or the business owner, depending on who runs the job. Sole traders can keep it as their own record.
