Close out a corrective action with evidence

Short answer

A corrective action is closed when someone other than the fixer has checked the fix and the evidence is on the record. Name an owner and a due date, fix the cause, photograph the result and record who verified it. Anything less is a hope, and the same hazard comes back.

What closed means

Plenty of actions are closed because someone said “done” at the Monday meeting. Then a worker finds the same missing guard on Thursday. Closed means checked. Someone looked, and the record shows what they saw.

Start with an action someone can finish

“Improve housekeeping” can’t be closed, because nobody knows when it’s finished. “Clear the east stair of offcuts and fit a bin at the landing by Friday” can. A good action says what, where, who and by when.

  • What exactly will change, in one sentence
  • Where, with a level, area or gridline
  • One owner, by name, not a team
  • A due date that suits the risk
  • How you’ll know it worked

Evidence that holds up

Evidence is whatever a stranger could look at and agree the job is done. Take it from the same spot as the original report, so the before and after line up.

  • A photo from the original angle, with the time on it
  • The hire docket or delivery note for the new guard or barrier
  • The signed permit or inspection tag
  • The toolbox talk sign-on where the change was explained
  • A test result or measurement if the fix was about noise, dust or electrical safety

Someone else checks

The person who made the fix is the worst person to confirm it. They know what they meant to do. A supervisor or the original reporter has fresh eyes. Walk the spot, look at the photo and sign it off with a name and a time.

When the fix will be late

Sometimes the part doesn’t arrive or the weather wins. Don’t quietly move the date. Write down why, set the new date and put an interim control in place, like a barrier, a sign or a different route. Then the record shows the hazard was managed while it waited.

An example

Take a loose tread on the east stair, reported on Monday with a photo. The supervisor assigns it to the carpenter for Wednesday. On Wednesday the tread is refitted and the carpenter takes a photo from the same step. On Thursday the supervisor walks the stair, checks it against the photo and signs it off.

Four people touched it. The record shows each step, in order. If anyone asks in six months, the answer is a minute away.

Close the loop with the person who reported it

The worker who raised it wants to know it went somewhere. A one-line reply does it: “Fixed on Thursday, here’s the photo.” It takes a minute and it’s the main reason the next report gets made. Our guide on getting workers to report hazards goes into this.

In Haznote

A manager confirms the report, sets a priority and assigns the action. The close-out sits on the same record as the original report, with the photos and timestamps. The worker’s words, the AI suggestion and the manager’s decision stay in order, and incidents are marked, not deleted.

Questions

Plain answers.

How long should a corrective action stay open?

It depends on the risk. A missing edge barrier is a today job. A drainage fix may take a week. Set the date when you assign it, and record any change with a reason.

What if the reporter has left site?

Another supervisor checks the fix against the original photo and signs it off. The record shows who looked and when.

Do I need a photo every time?

Not always. A signed permit or a delivery docket can be the evidence. Use what a stranger could check later.

Who signs off a high-priority action?

Someone with the authority to say the hazard is controlled, who isn’t the person who made the fix. On most sites that’s the site manager or a supervisor.
